The Evaluation Process

  1. Referral Inquiry
    Most evaluations begin with a phone call or email—often from an attorney, adult child, care manager, or Office of Aging.
  2. Intake and Overview
    We will provide a clear overview of the process and fees, then schedule a 45–60 minute intake call with Dr. Nebel to review concerns and gather case details.
  3. Fee Agreement and Invoice
    A flat-rate fee is required upfront to secure your evaluation appointment (due within 48 hours of invoice). This covers the full evaluation, report writing, and care coordination. Legal prep, extensive document review, expedited reports, and court services are billed separately.
  4. Record Collection
    A release of information is sent for medical records. Once received, records are scanned into the chart and forwarded to the individual’s provider for clinical background.
  5. In-Home Evaluation
    Dr. Nebel visits the individual’s home to conduct the capacity evaluation using a standardized clinical framework and nuanced observational insights.
  6. Report Generation
    A detailed, physician-signed report is delivered to the requesting party—usually an attorney, family member, or care manager.
  7.  Additional Legal Support (If Needed)
    If court testimony, legal coordination, or expert consultation is required, Dr. Nebel provides additional consultation and testimony services.
